We work with primary care optometrists, physical therapists, occupational therapists, neuropsychologists, concussion specialists, teachers, and physicians to address concerns related to our patients' visual system. We welcome your collaboration.

To schedule an initial evaluation for your patient, please complete our online form below and our office will contact them to set up an appointment.

We are happy to provide your office with complimentary printed materials about our services to share with your patients. To order a supply of brochures, you'll need a password to access our secure online portal. Please fill out the form below, and once approved, we'll send you a password.
